Cloud Subscription or On-Premise? How to Choose the Right Deployment for Your Hospital

20 August 2026 · 2 min read

One of the first questions hospitals ask us is whether they should run Doctopath as a cloud subscription or an on-premise install. There isn't a single right answer — it depends on your infrastructure, budget, and internal policies. Here's how to think it through.

When a cloud subscription makes sense

  • You don't have a dedicated IT team. Cloud subscriptions mean we handle updates, backups, and uptime, so your staff isn't responsible for server maintenance.
  • You have multiple locations. A cloud setup makes it straightforward for staff to access the same records from any branch.
  • You want to get started quickly. There's no server to procure or configure — your team can start using the system as soon as accounts are set up.
  • Predictable monthly cost works better for your budget than a larger upfront investment.

When an on-premise install makes sense

  • Your hospital has an internal IT policy that requires patient data to stay on infrastructure you control directly.
  • You already have server infrastructure and IT staff who can maintain it.
  • You operate in an area with unreliable internet connectivity, where a locally hosted system avoids dependence on a stable connection for daily operations.
  • You prefer a one-time license over an ongoing subscription.

It doesn't have to be permanent

Plenty of hospitals start with a cloud subscription while they're small, then move to on-premise once they've scaled up and built out their own IT infrastructure — or the other way around, starting on-premise and moving to cloud once they open additional branches.

Because Doctopath supports both deployment models on the same underlying system, that transition doesn't mean starting over with new software.

If you're weighing this decision for your own hospital, reach out to our team — we can walk through your specific setup and recommend which model fits best.
